Ang simbahan ay itinayo noong ika-15 siglo sa ilalim ng Franciscan Missionary na si Fray Francisco dela Cruz y Oropesa na tumagos sa masukal na kagubatan ng Caramoan Peninsula noong 1867. Pagkatapos ay itinatag niya ang isang pamayanang Kristiyano sa isang lugar na tinatawag na Balwarte. Sa turnover ng settlement na ito sa Sagrada Mitra para sa administrasyon noong 1696, isang kapilya ang itinayo mula sa mga katutubong materyales tulad ng nipa, kawayan at kahoy.
